Personalized Nursing Care LLC offers two distinct services. One provides skilled nursing care in the home. The other provides experienced RN guidance when healthcare has become difficult to understand, organize, or navigate.
Nursing Support Service
Experienced RN guidance for individuals and families who need help understanding, organizing, and navigating complex healthcare needs.
Nursing Support Service is designed for individuals and families who need help making sense of complex healthcare needs but may not require ongoing skilled nursing care.
PNC provides experienced nursing perspective to help bring clarity and organization to situations that can otherwise feel overwhelming.
Services may include:

Reviewing medical information and helping organize a complicated health history

Helping individuals and families understand their current healthcare situation

Preparing questions and information for appointments with physicians or other healthcare providers

Helping organize medications, treatments, appointments, and other healthcare information

Identifying areas that may need further discussion with the individual's healthcare providers

Providing practical guidance as healthcare needs change
Nursing Support Service is provided on an hourly basis and is tailored to the individual's or family's needs.
Private Duty Nursing
One-on-one skilled nursing care in the home for individuals whose medical needs require ongoing professional nursing care.
Private Duty Nursing provides individualized registered nursing care for individuals whose medical needs require ongoing professional nursing care in the home.
Care is provided directly by an experienced registered nurse and is based on the individual's needs and physician-directed plan of care.
